The Cancer Survivorship Checklist was developed by the National Coalition for Cancer Survivorship, now Cancer Nation, to guide patients and caregivers to important information throughout the cancer journey. Users can customize the checklist by age and cancer type, with an adolescent and young adult–specific option for ages 13–39.
